Supporting Documents

Please collect all applicable documents that are listed below,

You will need some of them to complete the program.

Also, please save these documents in a file, as you will need to send copies of these documents to the IRS if you qualify for Currently Not Collectable or Partial Pay, which will be more fully explained at the end of the program.

PROFILE PAGE

☐    Copy of your most recently filed IRS Form 1040, and all attached Schedules, especially the Schedule C, for business and Schedule E, for Real Estate. 

☐    Copies of your most recent IRS tax notices, for each year due, showing the amounts due and tax year.

ASSETS PAGE

☐   Copies of individual bank statements for the three most recent months.

☐   Copies of credit card statements from all your credit cards, for 3 recent months. 

☐    Copies of the most recent pay stub, earnings statement, etc., from each employer, for both you and your wife, if married.  You can request a copy from your Personnel Department.

☐   Copies of the most recent statement from lender(s) on loans such as mortgages, second mortgages, vehicles, showing monthly payments, loan payoffs, and balances. If these amounts are paid from your bank account, you can use your bank account statement as the proof of each of these payments. If any payments are made by your credit card, you can use your credit card statement as proof of these payments.

☐   Copies of the most recent statement, from all other sources of income such as pensions, Social Security, rental income, interest and dividends (including any received from a related partnership, corporation, LLC, LLP, etc.), court order for child support, alimony, and rent subsidies.  If these amounts are directly deposited into your bank account, you can use your bank account statement as the proof of each of these payments.

☐   Copies of the most recent statement for each investment and retirement account.

☐   Proof of real estate taxes paid for one month.  If you paid yearly, divide that number by 12.  If you paid 2 times per year, divide that number by 6. If these amounts are paid from your bank account, you can use your bank account statement as the proof of each of these payments. If any payments are made by your credit card, you can use your credit card statement as proof of these payments.

EXPENSES PAGE:

☐   Proof of rental payment paid for the past month. If this amount is paid from your bank account, you can use your bank account statement as the proof of such payment. If any payments are made by your credit card, you can use your credit card statement as proof of that payment. 

☐    Copies of your most recent utility bills and proof of payment for:  Gas/Oil, Electric, Water, Sewer, Trash, Phone, Cable, Cell Phone, Internet. You do not need these statements, if these bills are paid from your bank account or your credit card. You can use your bank account statement as the proof of each of these payments, or you can use your credit card statement as proof of these payments.  If you paid by cash, you would need a copy of the bill from each utility as proof of the payment.

 ☐  Copies of Statements and proof of Payment for: Monthly Auto Payment or Lease Payments, Auto Repairs and Maintenance, Fuel, Parking, Licenses, Public Transportation. If these amounts are paid out of your bank account, you can use your bank account statement as the proof of each of these payments. If any payments are made by your credit card, you can use your credit card statement as proof of these payments.

☐    Copies of Statements and proof of payment for:  Monthly Health, Dental or Vision Insurances, Co-Pay amounts, Prescription payment or Co-Pay. You can ask your health provider and the pharmacy for a printout of these expenses and amounts paid. If these amounts are paid out of  your bank account, you can use your bank account statement as the proof of each of these payments. If any payments are made by your credit card, you can use your credit card statement as proof of these payments.

☐   Other deductions allowed by the IRS. Get Copies of the most recent statement and proof of Monthly payment for:  Child Care expenses, Estimated Tax Payments, Term Life insurance, Retirement (such as 401K or pension), Union Dues, Student Loans, Court Ordered Child Support or Alimony or Court Ordered Other payments.  If these amounts are paid from your bank account, you can use your bank account statement as the proof of each of these payments. If any payments are made by your credit card, you can use your credit card statement as proof of these payments.

☐   Copy of the State/Local Tax Liability showing total delinquent state/local taxes and amount of monthly payments, if you have an Installment Agreement set up by such entity.

☐   Documentation to support any special circumstances described in the “Explanation of Circumstances” such as your need for additional food, medical and transportation, due to illness. Request a letter from your health provider(s) to support that you have an illness or illnesses that require special food, extra medical costs and transportation to and from medical appointments.

Business and Independent Contractor Page:

☐   If you operate a business, copies of the three most recent statements for each business bank account.

☐   Profit & Loss Statement for the past 3 or 6 month;  OR you may use a copy of your Schedule C, line 29, from your most recently filed tax return, instead of completing a new Profit & Loss Statement.  If your income has gone down since you filed your last tax return, then you should complete a new Profit & Loss Statement for the current year.

☐   List of all outstanding debt:  list the person/entity owed money, date it was due, and amount.

☐   List of clients that owe your business money for the past year, or Notes Receivable, if applicable
